Anton LeVay Killed Jayne Mansfield?

Doesn't it seem rather odd that once Anton LaVey put a curse on Sam Brody, and actually said that Brody would die in a car accident, that he was in six car accidents between 1966 - 1967, the final one killing him and Jayne Mansfield?

What is the other headstone then?

It's not a headstone on an actual grave. It's a cenotaph (a memorial / commemorative monument or empty tomb only). The current Hollywood Forever Cemetery is the latter-day incarnation of the once-prestigious Hollywood Memorial Park cemetery.
Jayne Mansfield (1933–1967), actress (she has a cenotaph; she is buried in Fairview Cemetery in Pen Argyl, Pennsylvania) (aged 34)

Gratuitous Fun Facts:
A total of two other famed movie figures have cenotaphs rather than actual graves there:
- Hattie McDaniel (Oscar winner as "Mammy" in Gone With the Wind; denied burial in 1952 because the cemetery was segregated)
- Terry (the dog who played Toto in The Wizard of Oz).

Perhaps the later birthdate on the cenotaph reflects Hollywood's habit of knocking a few years off ages? :cool:
 
I think this pretty much explains it. Back in the Fifties / Sixties it was sort of a running gag that stars would admit only to an age ending in "9" at the end of whatever decade they preferred. For example, it was a running joke that venerable comedian Jack Benny insisted he was 39.

Mansfield's heyday was in her twenties, so I suspect someone decided to immortalize her as if she'd died in her best decade.
The granite slab, flat on the ground on the northeast side of the cemetery, is along the north edge of the small lake. It features an inlaid color photo of blonde bombshell Jayne in a white dress. Unsurprisingly, while the grave marker in Pen Argyl marks her birth year as 1933, the cenotaph uses the "I work in sexist/ageist Hollywood"-adjusted year of 1938.
